How to fill Z83 step-by-step
Use the new Z83 form and complete every section. Do not leave blank spaces — write N/A if something does not apply. Unsigned or incomplete forms can be rejected.
Section A: Job details
Copy exactly from the advert:
- Job title
- Department name
- Reference number
- When you can start
Section B: Personal details
Write your full names, ID/passport number, citizenship, race, gender, disability status, and criminal record info honestly.
Section C: Contact details
Add your phone number, email, postal address, and choose how they should contact you. Make sure the number works.
Section D: Languages
Write languages you know, then rate each one for speak / read / write: Good, Fair, or Poor.
Section E: Qualifications
Start with your highest qualification first, for example:
- Matric
- Certificate
- Diploma
- Degree
Section F: Work experience
List jobs from most recent to oldest. Include employer, job title, dates, and reason for leaving. Attach your CV too.
Section G: References
Use people who know your work/school performance. Add name, relationship, phone number, and email if available.
Declaration — very important
Sign and date the form. If you forget this, your application can be disqualified.
